It's just means this speaker tired of seeing one more pimple.
他表示說話者已經疲於看到又一顆面皰了。

"Not another + something " is very common in spoken English
這種表示法在英文口語中非常常見。

例如打開email發現又中毒,你可能會說:
Not another virus!

例如整天有人打電話用任何理由請你做事,此時電話又再次響起,你可能會說:
Not another phone call!

他因為車禍而留下了疤痕。

scar 亦指心理上留下疤痕
Being abandoned by his mother scarred him for life.
被母親拋棄的事實讓他受傷一輩子。
